Ketamine is a racemic compound made up of equal parts of its two enantiomers, (S)-ketamine and (R)-ketamine. “What on earth does that mean?” Think of it this way, the ketamine we normally use in an IV for general anesthesia and in low doses to treat mental health disorders and chronic pain is made up of a pair of molecules. These two molecules are mirror images of one another, much like our hands. While they are mirror images, the molecules are not identical. The esketamine molecule is actually more potent at the NMDA receptor which is believed to be the main site of action of ketamine. Johnson and Johnson isolated the “left hand” of ketamine or, esketamine, and created the nasal spray, Spravato.

Spravato treatment at Imagine Healthcare begins with a consultation to determine if you are a candidate. If you are eligible, treatment will be administered under the supervision of our expert medical team. During treatment, you will self-administer the nasal spray and be monitored for two hours to ensure your safety and comfort.

  • To qualify for Spravato treatment, you must meet the following criteria:

    • Be at least 18 years old

    • Have tried multiple antidepressants from different classes with no success

    • Be diagnosed with treatment-resistant depression (TRD) or major depressive disorder with acute suicidal ideation or behavior (MDSI)

    • Receive treatment at a certified Spravato treatment center

  • Some patients notice symptom relief within hours, while others may see gradual improvements over weeks.

  • Mild side effects include dizziness, nausea, sedation, or temporary blood pressure changes—our team monitors your safety.

  • Patients start with twice-weekly sessions for 4 weeks then continue weekly treatments for several months. Frequency of treatments may then decrease over time based on progress.
